What is a VaxCard?

VaxCard is a durable and waterproof version of your personal CDC Vaccination Record card. It is wallet-sized so that you have it when you need it, and is lanyard ready if you need to display it.

How do I ensure that the image of my CDC Vaccination Record card is clear and highly legible on my VaxCard?

When ordering your VaxCard you will be presented with a preview of what your card will look like. This preview is an exact representation of what your VaxCard will look like when printed. If the preview is not clear and legible, click “rescan” and try to take your picture again. Cleaning your camera lens, using a dark background, using good lighting, and holding your phone still in portrait mode directly over your card will all help to ensure a high quality photo is taken

My original CDC Vaccination Record card was damaged/lost/never received, can I still order a VaxCard?

If your original CDC Vaccination Record is damaged to the point that a clear photo cannot be taken, or if your CDC Vaccination Record was lost or never received, then you will need to obtain a replacement CDC Vaccination Record before ordering our VaxCard.

Obtaining a replacement CDC Vaccination Record:

If you received your vaccine at a pharmacy or through your healthcare provider, you can reach out to that pharmacy location, clinic, or hospital to receive a card replacement. You can also contact your vaccine provider if for any reason you did not receive a vaccine card.

If you received your vaccine through a large pop-up site or mass vaccination event at a stadium, a convention center, or another location that is no longer in operation, you may need to track down a replacement card through the health department in charge of running that vaccination site. You can also contact your state’s health department for additional guidance.

Now that I have a VaxCard, what should I do with my original CDC Vaccination Record card?

One of the many benefits of having a VaxCard is that you no longer need to carry your original card with you! We recommend storing your original CDC Vaccination Record in a secure place as you may need it in the event that a booster shot is needed or if you need to order a replacement VaxCard.

How is my personal information secured?

We take customer privacy and security very seriously. We use industry-standard best practices to ensure a high level of security and safeguarding of customer data. VaxCard uses TLS (SSL) to force HTTPS for all web interactions including vaxcard.com and go.vaxcard.com. All data, including personal information and vaccine card images, are encrypted at rest and while being uploaded to our servers. We achieve this level of security by leveraging world-class, HIPAA-compliant cloud-hosting and printing partners. How do I order a VaxCard?

Ordering a VaxCard is really simple and typically takes less than a minute!
First, place your original paper CDC Vaccination Record card on a dark flat surface background with good lighting.
From a phone or tablet with a camera, go to vaxcard.com and click “Order your VaxCard”. Enter your phone number, agree to the terms, and click “Scan CDC Vaccine Card”.
Depending on the browser that you are using, you may be prompted to allow the use of your camera. You must allow camera use in order to take a photo of your vaccine card. After taking the photo, this permission is no longer in effect.
Now your camera will open and prompt you to scan your vaccine card. Hold your phone/tablet directly over your card. A photo will automatically be taken once everything is aligned and in focus.
Next, a preview of your card will appear. This preview is an exact representation of what your printed VaxCard will look like. If the preview is not clear and legible, click “rescan” and try to take your picture again. Cleaning your camera lens, using a dark background, using good lighting, and holding your phone still in portrait mode directly over your card will all help to ensure a high quality photo is taken.
Finally, enter your payment and shipping details and submit your order. That’s it!

What if booster shots are needed and I want a new VaxCard showing that I have received my booster shot?

If booster shots become necessary, we are planning to offer existing VaxCard customers a discount on an updated VaxCard. We will email VaxCard customers with discount information if boosters become necessary.

In the meantime, if you need an updated VaxCard for any reason, you can always place a new order.

My original CDC Vaccination Record card was damaged/lost/never received, can I still order a VaxCard?

If your original CDC Vaccination Record is damaged to the point that a clear photo cannot be taken, or if your CDC Vaccination Record was lost or never received, then you will need to obtain a replacement CDC Vaccination Record before ordering our VaxCard.

Unfortunately, at this time we are only producing VaxCards for original CDC Vaccination Record cards.

Technical Support

The camera tool is not working. It just says “Initializing”. What do I do?

First, if you were prompted to allow camera permissions and you did not accept them, this is probably the issue. Please re-attempt your order and allow camera permissions.

If you are not prompted to allow camera permissions, or if you have allowed them and the camera tool is still not working, please attempt to use a different mobile web browser when accessing vaxcard.com. Chrome, Firefox and Safari mobile browsers have worked well in our testing. We have observed some issues when using the Samsung mobile internet browser. You may also try using a different device altogether as long as it has a camera.

The preview of my VaxCard is not in focus. What do I do?

If the preview of your card is still not in focus, please try using a different phone or tablet

I received my VaxCard and it is not legible. What do I do?

Please contact info@vaxcard.com so that we can look into and resolve the issue.

Can I order a VaxCard without a smart phone or tablet?

Unfortunately, a mobile device with a web browser and camera is required to order a VaxCard. If you have a friend or family member with a suitable device, see if you can borrow it. It will only take a minute to order your VaxCard!

Shipping & Delivery

When will I receive my VaxCard?

Please allow up to 10 business days for us to produce and ship your card. From there, shipping typically takes 3 business days.

Can I track my order?

At this time, we are not able to provide tracking information, but we will send an email notification to you once your card has shipped.

How do I make changes to my order (e.g. shipping address)?

To make changes to your order please email info@vaxcard.com. In your message, please include your full name, the phone number used to place your order, your receipt number (if you received one), and the order change that is needed.

Where do you ship?

We ship anywhere in the United States, including Alaska, Hawaii, Puerto Rico, and the U.S. Virgin Islands.

How will my VaxCard be shipped?

Your VaxCard will be shipped by USPS First-Class Mail in a security envelope. Every VaxCard you order will be shipped in its own envelope. Please be on the lookout for multiple envelopes if you order multiple cards.
